2012-13 Registration Form & Medical Release :
To sign this form, you will be asked to create a digital ID (if you do not already have one). To submit a PDF of this form to Children's Ministries, click the button at the bottom of the second page. (It may say it is only sending the information, but we will receive the entire form.)

Each year we are asking for parents' help in keeping their child's information current.  Up-to-date contact information allows us to communicate with parents through newsletter and postcard mailings .Those new to Children's Ministries at any time of year should also submit this registration form.
